Plot[]
The CIA team's hunt for Perseus has led them to the KGB headquarters in Moscow, along with the cache of sleeper agent being stored inside a nuclear bunker underground. With only one way in and out of the bunker, Russell Adler decides to call for Dimitri Belikov, their mole in the KGB, to get them inside. Sims is skeptical that Belikov will honor his word, but Adler insists that he will get them through.
Adler and "Bell" are sent to undertake the mission, with Belikov watching their movements. Belikov finds that the security measures have been increased, just as a soldier informs him that Gorbachev has requested him for a high-level meeting. Upon arriving at the meeting, Gorbachev states that there is a mole among them, and that they must be found. As a safety precaution, all but General Charkov's bunker access has been revoked. Belikov disables the building's CCTV before making his move to acquire a key:
Belikov decides to take Charkov's key by poisoning him with Nova-6. He enters Kravchenko's office and steals a bottle of Nova-6, before requesting a meeting with Charkov's receptionist under the guise of having some tea. The receptionist brings him into Charkov's office, where Charkov is busy talking to someone on a phone as the receptionist pours tea for the two. Belikov pours the Nova-6 into one of the cups, but Charkov takes the non-poisonous tea; fortunately, Belikov is able to swap the two cups as Charkov is distracted. Charkov then states that he already knows that Belikov is the mole and that he wants his card key, but dies from ingesting the poison. Belikov takes the card key and leaves through a side office.
Knowing that Charkov is in his office, Belikov sneaks in the the War Room next to it before entering the room and killing Charkov with his knife to acquire the bunker key.
Belikov decides to create a card key of his own. All the card key blanks are inside the armory. Belikov has various options to choose from to enter, either by bribing the guard with a cigar, showing him the necessary paperwork or eliminating him as the cameras have been disabled. The guard lets Belikov in, who takes one of the blanks and programs it under Charkov's credentials.
Belikov decides to frame Charkov, having realized that he is nearly close to being compromised as they are tracking his communications. He makes his way to the communications room, killing the guards there. He passes off the communications as Charkov's, printing out one document as evidence to support his claims. With the file on hand, he then makes his way to Kravchenko and gives him the fake documents. Kravchenko is surprised that Charkov was the mole, but accepts the fake document as proof. As Charkov arrives, Kravchenko orders him to forfeit his possessions, which Charkov reluctantly does. Belikov swipes the card key from the counter while no one is looking. Charkov attempts to convince Kravchenko Belikov is the mole, but the fake document is already proof for Kravchenko. Charkov is then incarcerated in the prison underneath the building.
Aware that Charkov is interrogating a prisoner named Ivanov, Belikov makes his way to the questioning room. Ivanov refuses to cooperate, prompting Belikov to find Ivanov's file. Upon getting his file and learning of the situation, Belikov returns to the questioning room, convincing Ivanov to help him by killing Charkov, loosening his handcuffs. Belikov leaves the room just before Charkov arrives, who enters the room and taunts Ivanov about his daughter. Ivanov becomes enraged and kills Charkov by stabbing him in the neck with a pencil, before the guard standing outside rushes in and bashes Ivanov with the butt of his rifle. Belikov then orders the guard to stand put outside as he pickpockets the card key.
While acquiring the key, Belikov can take out Richter.
After acquiring the key, Belikov makes his way to the basement and enters the furnace room, where Adler and "Bell" are waiting for him. As they need disguises, Belikov calls for two officers to investigate an open door. The two then ambush and kill the guards, taking their uniforms as a disguise before making their way to the bunker area.
Adler and "Bell" then make their way, gear in tow, eventually arriving at the entrance. They try to bypass through a side door, but are caught by a guard, who insists that everyone must go through the checkpoint. Just as it seems that their mission will be compromised, Belikov comes through once again to get them through, where they take the elevator to the bunker. Before the elevator doors close, Zakhaev enters and asks the two who they are and where they are going.
"Bell" chokeholds Zakhaev before Adler sucker punches him out cold.
If not knocked out and "Bell" states that they are reporting to Sobol, Zakhaev does not suspect the two of anything and stops at his destination, stating that he has a meeting with Sobol and promises to give him their regards as he leaves.
Zakhaev pistol whips Adler before pointing his pistol at "Bell", realizing that they are imposters. However, his pistol whip simply knocks Adler to the ground, who then gets back up on his feet and the duo beat Zakhaev up, leaving him unconscious. After hiding him, they make their way to the bunker.
Regardless of how Zakhaev is dealt with, the two discard their disguises and begin equipping themselves. They engage in a massive fight to get to the vault; halfway through, Adler has "Bell" set up a secondary plan by placing a gas can in the ventilation system if their escape is compromised. Entering the vault, "Bell" quickly copies a list of the sleeper agents in the United States as the two attempt to escape before reinforcements arrive.
As the pair fight their way out of the KGB headquarters, the two run into a group of soldiers with a captive Belikov, having realized that he was the mole. Adler, refusing to let Belikov die, has "Bell" activate the gas to save him. Belikov, grateful for the rescue, informs the two that the Soviets are locking the building down. They take a brief detour to the armory to arm and equip themselves with Heavy Armor and escape in their get-away vehicle with the intelligence in tow.

- The phrase "We are in grave danger from the capitalists. Our collective, our very way of life is at risk." said by Mikhail Gorbachev to is a nod to the conversation between John F. Kennedy and Alex Mason during the mission U.S.D.D. in Call of Duty: Black Ops.
- If the player chooses to kill Charkov by sneaking into his office through the War Room, they will be able to pick up his body. They can also do this if he is poisoned. If the player later chooses to drop off his body outside the War Room, the body will still be there and can be picked up by the player playing as "Bell". Walking around the Lubyanka Building while holding his body will not, strangely, alert any Soviet guards.
- If Charkov’s body is dropped in the elevator before going down, it will despawn after Zakhaev enters the elevator.
- The player will have unlimited ammo and grenades after entering Lazar’s car.
- At the point where Belikov and Adler are talking through the door in the basement, Bell can briefly be seen standing in the background. Male Bell's third person model appears to be Hudson but without his leather jacket.
- While the player is Belikov, the door that will later be used to escape while playing as "Bell" is barred and unusable. However in some instances, it can be opened while playing as Belikov through the bars to reveal the unfinished escape route, including a red strobing light. The bars still prevent player travel. The trigger for this seems to be playing through the level in it's entirety, then selecting "Restart Level" before going up the elevator.
- Bodies places behind lockers can be seen despawning if one looks through the gap between the doors shortly after hiding them.
- When player controls Belikov, Russians speak English, but when player controls "Bell", Russians speak Russian.
- If the player knock Zakhaev out his body will disappear after arriving to the bunker.
