Zombie Glitches

I was playing zombies with a few friends on Black Ops. I was playing Ascension. We were using the box and got a teddy bear. We looked at the tv and it was near the lander near Speed Cola. We went and it wasn't there. We looked at all the other mystery box spots and it wasn't at any of them. So we had no mystery box. We could only get it on firesale. And my friend picked up the death machine power-up and he couldn't use it. He was still using his normal gun. Has any of this ever happened to you?
